Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post‑discharge. Ideal candidates thrive in interdisciplinary environments and are committed to improving patient outcomes through holistic, evidence‑based care.
Key Responsibilities
Coordinate psycho-social support for patients during hospitalization
Develop and implement personalized discharge plans
Assess patient needs related to care coordination, resources, and services
Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction
Facilitate access to community resources to support recovery and continuity of care
Ensure compliance with hospital policies, regulatory standards, and evidence-based practices
Qualifications
Education:
Bachelor’s degree in Social Work required; Master’s degree preferred.
Experience:
Minimum two years in acute care, SNF, physician offices, or community health settings.
Licensure/Certification:
LBSW license in Maryland required; Must obtain care/case management certification within five years of hire.
Skills & Abilities:
Strong communication and multitasking skills; Familiarity with EMRs; Knowledge of disease processes, regulatory requirements, and evidence-based care; Understanding of community resources, provider networks, and the revenue cycle; Ability to build relationships across clinical and community settings.
