Join DaVita Kidney Care to help patients on dialysis navigate the physical, emotional, and financial challenges of End Stage Renal Disease. As a vital member of the core care team you will advocate for patients’ rights, provide counseling, and coordinate resources to support a smooth transition to life on dialysis. Base pay range
$74,900.00/yr – $118,000.00/yr Responsibilities
Build meaningful and long‑term relationships with patients and their families in an intimate outpatient setting. Advocate for patients’ rights and responsibilities and help them understand and manage the demands of End Stage Renal Disease. Coordinate care, resources, and support services across the interdisciplinary team to improve patient outcomes. Qualifications
Meets all state required regulations to practice in the dialysis setting. Master’s degree in Social Work (MSW) required with a specialization in clinical practice. Demonstrated knowledge of government and private insurance programs. Basic computer skills in MS Word, Excel, PowerPoint and Outlook; proficiency with DaVita-specific applications within 60 days.
