Job Description Summary

Under the general supervision of the Transplant Program Manager, the candidate provides professional social work to facilitate care for transplant patients. Responsibilities include care coordination of the bio-psychosocial needs of transplant patients and leadership in development, implementation, and coordination of treatment plans. In collaboration with other health care professionals, the Master Level Social Worker provides a variety of comprehensive services to maximize quality and cost‑effective healthcare along the continuum of care appropriate to the transplant patient’s needs.

Provides transplant social work services for facilitating solutions to medically related psychosocial/environmental problems of patients and families. Ensures continuity of patient care by implementing interventions such as initial assessment, identifying mental health disorders, brief behavioral health interventions, information and referral during hospitalization and/or outpatient treatment. May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients.

Education and Credentials

Master’s degree in a social work or social welfare program accredited by the Council on Social Work Education. MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
