Job Summary The Social Worker at Research Medical Center utilizes professional social work knowledge, relationship skills and interdisciplinary collaboration to assess and intervene in psychosocial issues significant to patient/families and health care team's management of illness, injury and disability. The role coordinates discharge planning; provides information and referral to appropriate community agencies; addresses questions about financial and other issues; completes appropriate paperwork for post‑hospitalization transfer and makes appropriate entries into the medical record.

Qualifications

Masters Degree in Social Work required

Minimum 1 year hospital experience
