Job Description
Ensures compliance with disease‑specific standards of care and other regulatory bodies.
Participates in conferences and teleconferences for readmission reduction programs. Compiles data for audits and accreditation.
Coordinates patient care post‑discharge for high‑risk readmission patients.
Stays updated on regulatory requirements. Engages in discussions to reduce readmissions. Gathers and reports data accurately for audits.
Manages follow‑up care to prevent readmissions. Develops and implements strategies to reduce readmissions.
Qualification Requirements
Bachelor's degree (required)
Technical/Vocational School education (required)
Advanced Cardiac Life Support Certification (ACLS)
Residency Quality Improvement (RQI) certification
Advanced Practice Registered Nurse (APRN) – EV Accredited
Basic Life Support – CPR Certification (BLS) – RQI Resuscitation Quality Improvement
Pay Range $68,132.50 - $119,520.35
