Credentials Required : Bachelor’s in Social Work or related and approved Human Services field
Special Skills/Knowledge Required : Computer and documentation skills; background in DDID preferred.
Primary Functions of Job
Conduct monthly home visit with participants to assure health, safety, and welfare are being maintained.
Provide training and assistance to help participants with Plan of Care and Budget.
Complete assessments and reassessments for participants in a timely manner.
Monitor service provision and assist in the development and completion of a person-centered plan of care
Ensure PDS employee timesheets meet regulatory requirements.
