Credentials Required:
Bachelor’s degree in approved Human Services field (Social Work, Psychology, Sociology, etc.) with one (1) year post graduate work with children.
Special Skills/knowledge:
good communication skills, maintain client related goal data, including case notes, outreach, and relationships with community partners.
Primary Functions:
Coordinate, link, monitor, and assist clients and families on caseload to identified services and needs.
Collaborate with community partners and service team members.
Work among and facilitate a service team.
Identify and link clients and families to community resources.
Maintain knowledge of community resources and how to access resources.
Complete documentation and medical record requirements
Shift/Days and Work Hours:
Monday-Friday, 8:00am – 4:30pm
Salary:
$17.50 hour
