Salary: $6,257 – $8,322 monthly.
Job Description

Thurston County Public Health and Social Services (PHSS) is recruiting for two (2) Community Health Nurse II positions for our Community Health Division to serve families through the Nurse Family Partnership (NFP) program. As a Community Health Nurse II, you will deliver services to families within Thurston, Mason, and/or Lewis counties. Carrying a caseload of 25 active clients, you will conduct frequent home visits, build strong partnerships with families, and collaborate with a team to promote family health. Essential Functions
Assess physical, emotional, social, and environmental needs of women and their families as they relate to the NFP domains Formulate nursing diagnosis based on nursing assessment and client goals Assist women and their families in establishing goals and outcomes Provide education, support, and referral resources to assist women and their families in attaining their targeted goals Consult and collaborate with other professionals involved in providing services to women and families Plan home visits and evaluate client progress toward stated goals and NFP outcomes Actively engage in professional development to meet all NFP Nurse Home Visitor competency requirements Meet with NFP Nurse Supervisor weekly for clinical supervision, utilizing reflective practice in supervisory sessions Schedule joint home visits with NFP Nurse Supervisor every four months Attend and participate in bi‑weekly case conferences and team meetings Provide information to support staff for timely and accurate data input Participate in review and analysis of reports for achievements, areas for improvement, and quality improvement efforts Document appropriately and maintain confidentiality Perform other work as required or assigned, including a 24/7 response during public health emergencies and disease outbreak investigations Working Conditions
Work Environment – Quiet office setting in Olympia, Washington Schedule – Monday – Friday, 8:00 am – 5:00 pm, with potential alternative or compressed schedules and/or remote work Travel Requirements – Occasional travel to attend meetings, trainings, and onsite visits Customer Interactions – Daily interactions with county staff and the public Who We Are

Bachelor’s degree in nursing with coursework in community or public nursing. Completion of a BSN program within six months of hire may be considered. Two years of experience as a community health nurse or relevant experience in a hospital, medical clinic, public health, or related service. Equivalent combinations of education and experience are considered. Currently licensed as a Washington State Registered Nurse. A valid Washington State Driver’s License or appropriate accommodation.
