Job Description
Responsible for providing professional nursing for patients following established standards and practices. Assists physicians and staff with the daily coordination of patient care and patient access. Coordinates patient flow, schedules appointments, procedures, tests and referrals. Serves as preceptor for clinical staff.
Responsibilities
Clinical Practice and Care Management
Performs psychosocial/physiologic assessments that integrate changing data.
Formulates a plan of care based on trends of similar patients, including patient education and discharge planning.
Tailors and prioritizes care practice to individual needs, including cultural, ethical and spiritual needs.
Participates in routine transitional health‑care needs planning (treatment options, placement options, end‑of‑life options).
Responds to changes in patient’s needs and provides holistic care using independent clinical judgment.
Adapts educational programs to individual patients and families by modifying teaching strategies or content.
Ensures efficiency in care delivery: determines immediate priorities and provides solutions to problems.
Advocates actively for patient rights and resolves potential conflicts.
Detects variances from expected outcomes.
Evaluates patient outcomes and revises the plan of care accordingly.
Documents all patient‑care activities per documentation standards and assists the healthcare team with the documentation process.
Communication and Collaboration
Consistently communicates and collaborates with the health‑care team, patients, and families to maximize outcomes.
Delegates patient‑care activities appropriately and participates in staff assignments to achieve maximum productivity.
Maintains knowledge of unit initiatives and incorporates outcomes into practice.
Participates on unit teams, work groups, or committees, anticipating needs and voluntarily assisting others.
Quality
Demonstrates knowledge of unit‑based quality‑improvement initiatives.
Participates in quality‑improvement initiatives using standards, guidelines, and pathways for care delivery.
Incorporates evidence‑based practice and research findings into nursing practice to improve outcomes.
Professional Development
Acts as a clinical resource and mentor, displaying leadership behaviors.
Identifies personal learning needs based on self‑evaluation and progress from previous goals.
Accounts for all documentation and completion of all required continuing education and competencies.
Ensures practice improvements through lifelong learning responsibility.
Provides direction and guidance to others regarding practice and serves as a preceptor and mentor.
Qualifications
Graduate of an accredited school of nursing.
Proficient in written and verbal English communication and computer skills.
Current Louisiana State RN license and BLS certification.
Location: 2600 Tower Dr, Monroe, LA 71201.
Shift: Day; PRN (As Needed) schedule.
Department: Cardio Pulmonary Rehab – SFMC CHC Campus.
