Who You Are
Graduate of an accredited school of professional nursing
Maintains a current license with the Texas State Board of Nurse Examiners
Has a current card for Basic Cardiac Life Support (CPR – Healthcare Provider)
Experience with the MDS transmittal process and understanding of regulations impacting the MDS
In-depth experience and knowledge working with MDS computer software
Expertise in MDS processes and systems
Effective oral and written communication skills
Teamwork ability; assists all other department members when needed
Cooperative with other healthcare professionals and departments within the system
Practical problem‑solving skills when encountering limited standardization
Professional communication with internal and external contacts
Ability to maintain professional relationships with physicians, patients, visitors, and staff
Knowledge and implementation of infection control and universal precaution practices
Summary & Responsibilities The MDS Coordinator is responsible for overall supervision, coordination, maintenance, and timely completion of the MDS, care plans, and medical records for residents.
Verifies all MDS for both Medicare and Long‑Term residents are accurate and completed promptly
Supervises subordinate MDS coordinators
Ensures all assessments are accurate and timely
Oversees the monthly Triple Check meeting in accordance with accounting needs
Coordinates, conducts, and completes MDS, resident care plans, and conferences using the computerized system
Ensures accuracy and completeness of the entire medical record within HFCA and Medicare standards and guidelines
Ensures MDS electronic submission occurs per Texas regulations
Participates in family/resident care plan conferences
Maintains effective communication with other shifts and departments regarding medical record documentation
Notifies physicians and families of changes in resident condition
Encourages rehabilitative techniques to restore resident independence
Assists the Director of Nursing in developing and implementing medical records policies and procedures
Audits resident charts as assigned
Coordinates the restorative program
Prepares for and attends in‑service training and education sessions as assigned
Performs specific duties and responsibilities assigned by supervisor
Ensures timeliness of physicians’ visits
