What Youll Do
Conduct comprehensive resident assessments prior to move‑in, re‑entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.
Assist with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.
Establish and foster positive relationships with residents, families, associates, home health, hospice, and other external partners.
Coordinate and ensure proper health‑related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.
Provide direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.
