Responsibilities As a MDS Coordinator you will:
Develop, monitor, audit, and modify each resident’s care plan to meet their individual needs and goals.
Perform resident assessments and assist with the discharge process.
Provide your team with the knowledge and skills needed to deliver the high standard of care we are known for.
Maintain accurate and up‑to‑date documentation in compliance with regulatory requirements.
Qualifications
Unencumbered State of Iowa RN license.
Graduate of an accredited nursing program.
CPR certification.
Preferred: one year experience as an L.P.N./R.N. in a Skilled Nursing Facility and one year of experience as a C.N.A.; prior MDS Coordinator experience is also preferred.
Willing to invest in the right candidate.
