Overview
The Virtual Care Coordinator supports virtual patient care activities and promotes patient-focused care within the Mount Sinai Health System. This role performs remote monitoring duties related to patient safety and clinical needs, supports hospital-based telemedicine services, and monitors hospital environmental status. The Virtual Care Coordinator collaborates on implementing planned telehealth and remote patient monitoring programs, coordinates training with leadership, and participates in the development and execution of telehealth best practices. The role includes administrative tasks and requires clear communication and collaboration with the clinical team, patients, Digital Technology Partners, engineering, telehealth platform vendors, and other managers. This position shares accountability for clinical quality and safety outcomes.
