$57.30 per hour · Benefits are available to full-time employees after 90 days of employment · A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates · Mileage reimbursement is
$0.43 per mile Home Health Physical Therapist Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs No weekends or on-call required Coverage Area:
Martin County, Washington County, Tyrrell County, and Hyde County, NC HCHB knowledge is a plus The required availability for this position is
Monday–Friday, 8:00 AM – 5:00 PM Home Health Physical Therapist Responsibilities Perform evaluations and treatment of patients based on physician orders/referrals Complete comprehensive evaluations using appropriate tools and procedures Review patient records for diagnosis, medical history, precautions, and contraindications Establish measurable goals and develop treatment plans to maximize patient independence/function Re-assess treatment effectiveness, patient progress, and modify goals as needed Communicate patient needs and progress to the treatment team, physicians, patients, and family members Consult with medical providers regarding evaluation, treatment, progress, and discharge Recommend and assist in securing adaptive equipment to enhance rehabilitation Maintain and submit evaluation, treatment, and billing documentation per company, facility, and regulatory requirements Adhere to company policies, HIPAA, corporate compliance programs, professional standards, and applicable laws Supervise Home Health Physical Therapist Assistants and Aides in patient care and related activities Attend and participate in staff development programs and continuing education to maintain and enhance clinical skills
