Position Overview A leading healthcare organization is seeking an experienced

Clinical Pharmacist

to join its patient care team. This role focuses on clinical review, patient counseling, medication safety, and operational oversight within a collaborative, patient-centric environment. Clinical Duties Conduct comprehensive clinical reviews for each patient, including drug utilization assessments, counseling, compliance monitoring, interaction screening, and side effect evaluations. Document clinical findings clearly in the patient record to support coordinated care and follow-up. Counsel patients on new medications following established clinical management models. Complete REMS training and adhere to program-specific requirements. Collaborate with referring clinicians to support patient needs and coordinate tailored services. Act as a liaison among patients, caregivers, nurses, and prescribers to ensure high-quality, coordinated care. Provide subcutaneous administration teaching for patients and caregivers; coordinate nursing services when necessary. Participate in on-call rotation as directed. Process prescriptions accurately and verify appropriate discontinuation of therapy when patients complete or discontinue treatment. Support clinical management models for various disease states. Manage intake of new referrals and ensure excellent patient service and communication.

Operational Responsibilities Ensure strict adherence to policies related to government-funded programs such as Medicare and Medicaid. Dispense medications in accordance with state, federal, and board of pharmacy regulations. Verify accuracy of prescription order entry and perform final product quality checks. Support pharmacy technicians with claim adjudication and reimbursement accuracy. Monitor hazardous waste management and compliance within the pharmacy. Supervise pharmacy technicians and oversee aseptic technique standards. Utilize patient education resources and support reporting of adverse events and product quality complaints.

Professional Expectations Maintain continuing education requirements in accordance with state pharmacy laws. Support professional development of assigned pharmacy team members. Maintain a commitment to continuous learning and staying current with pharmacy practice trends. Participate in professional conferences and share relevant updates with internal teams.

Qualifications 3–5 years of post-graduate experience in pharmacy operations, clinical oversight, and direct patient care. Graduate of an accredited school of pharmacy. Active North Carolina pharmacist license in good standing (required). BCOP certification or willingness to obtain certification when eligible. Ability to obtain additional state licenses as needed. Strong verbal and written communication skills. Proficient in computer systems and documentation workflows. Knowledge of sterile technique, medication safety, and quality control procedures. Comfortable providing patient counseling by phone for extended periods (up to 8 hours/day).
