Connections Health Solutions is a leading provider of immediate-access behavioral health crisis care. We combine medical and recovery-oriented treatment to stabilize individuals in crisis and connect them to community resources for ongoing recovery.
Responsibilities
Facilitate admission intakes, treatment planning, discharge planning, obtain collateral and assist in legal processes.
Coordinate care and discharge planning for individuals, families, clinics, local providers, and other referral sources.
Maintain clinic records, complete signed treatment plans and document all services in the medical record.
Participate in interdisciplinary team meetings and develop individualized person‑centered treatment plans.
Assess and address gaps in service needs, coordinate referrals and provide daily programming/groups.
Maintain working knowledge of Virginia involuntary commitment process, DBHDS and DMAS behavioral health systems.
Participate in supervision, ongoing education, training and performance assessment as required.
Perform all duties as assigned.
Qualifications
Bachelor’s degree in behavioral health or related field.
Knowledge of the Commonwealth of Virginia behavioral health system of care and resources.
Qualified Mental Health Professional (QMHP).
Ability to perform Safe Clinch restraint techniques.
Must be fully vaccinated or exempt under company policy.
State of Virginia Fingerprint Clearance.
Pass Virginia DBHDS and VA DSS background checks.
It would be great if you had:
Master’s degree in behavioral health or related field.
Licensed Mental Health Professional (LMHP).
Experience working with children, youth, families, and adults with behavioral health issues.
Knowledge of and experience with the Virginia involuntary treatment process.
