Job Title: Field-Based Nurse Practitioner – AGNP / CNP | Wound Care Certification Paid + Independent Role
Compensation: $105,000 to $120,000 annually

We are hiring a field-based Nurse Practitioner (AGNP or CNP) who thrives in an independent, flexible environment. If you’re not yet certified in wound care, we’ll pay for it. This is a great fit for someone looking to grow their clinical specialty while making a direct impact on a vulnerable population. Key Responsibilities:
Visit assisted living, memory care, and senior communities to provide primary care Address wounds, chronic conditions, and preventative care in place Partner with care teams and families to ensure holistic support Keep accurate records and communicate clearly with all stakeholders Qualifications:
AGNP certification preferred Experience with geriatrics, family practice, or long-term care Must have a valid driver’s license and auto insurance Strong documentation, collaboration, and communication skills Military Occupational Classification (MOC) Codes:
