LTSS Service Coordinator (Case Manager) – CareBridge
Join us as an LTSS Service Coordinator (Case Manager) at CareBridge, a full‑time, entry‑level position focused on delivering high‑quality, trauma‑informed, person‑centered coordination for all members of the MyCare Ohio health plan.
Location This field‑based role requires traveling to client sites or designated locations as needed, with occasional office attendance for meetings or training. Candidates must be within a reasonable commuting distance of the posting location unless a legal accommodation is granted.
Key Responsibilities
Conduct face‑to‑face program assessments using various tools and are guided interview techniques.
Identify members with potential clinical needs and coordinate their care as the single point of contact with interdisciplinary teams.
Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities to ensure efficient use of long‑term services.
Document short and long‑term goals in collaboration with the member’s care team and assess eligibility for alternative services or waiver programs.
Serve as a mentor, preceptor, or subject‑matter expert for new staff and support process improvement initiatives.
Submit utilization/authorization requests with supporting documentation aligned with the care plan.
Report critical incidents to state and county agencies and assist with appeals, grievances, and audits.
Minimum Requirements
Bachelor’s degree (BA/BS) or equivalent experience with a social work agency; minimum 2 years of experience.
Preferred Skills, Capabilities, and Experiences
Case management experience with older adults or individuals with disabilities.
BA/BS in Health or Nursing preferred.
