Job Summary: Responsible for direct care coverage and supervision in the residential program and the implementation of meaningful day activities to all individuals.

Supervisory Responsibilities: None

Facilitate the implementation of meaningful day in your program

Daily Planning and Relaxation Meetings:

Ensure entire documents are completed for Mon – Fri from 9 AM to 3 PM

Ensure that sufficient copies are available

Purge information every three months into the black cabinet

Develop and prepare individualized meaningful day schedules for all individuals

Ensure daily schedules are being followed and filled out by all first shift staff

Periodically review schedules for accuracy and completeness, make changes as needed

Menus:

Ensure menus are following the correct template and are EO509 compliant

Post and update menus on the refrigerator or in a location designated by the Program Manager

File copies of all menus in the HR book

Order necessary supplies for meaningful day

Continuously pursue new activities and opportunities to keep schedule fresh

Offer training to staff on meaningful day at each monthly program staff meeting or more often as directed by Program Manager

Vocational Development and Job Procurement:

Involve individuals in social/recreational activities including:

Socialization and community integration

Development of meaningful interpersonal skills

Development of habilitations kills

Placement transitioning

Transportation and access of public transportation

Role Modeling

Leisure and recreation

Development of cooking, budgeting and shopping skills

Involve individuals in Functional Skill Development:

Development of communication skills

Personal hygiene and cleanliness of living area

Implement Activities of Daily Living. Individual Service Plan Objectives and Behavior Plans.

Provides transportation of individuals to scheduled appointments or social activities.

Record progress notes, behavioral data and log notes for each individual residing in the program.

Tracks data and progress on all identified areas of need.

Ensures the safety of the individuals and general public.

Models’ appropriate behavior for individuals residing in the program.

Engages in the upkeep of the residence.

Ensures adequate program coverage and supervision at all time.

Ensuring duties and responsibilities are carried out in a supportive and professional manner.

Attends appropriate meetings as directed.

Extended standing, sitting, running and lifting as needed to ensure safety.

Conduct physical restraints as needed.

Required Education and Experience:

High school diploma or equivalent.

Position requires criminal history background check.

PABC (physical restraints)

CPR/First Aid

MAP Certified

DDS Licensing and Certification required training

Preferred Education and Experience:

Preferred background in Human Services

Occasional Travel
