Service Coordinators with the Preadmission Screening & Resident Review (PASRR) program are Qualified Mental Health Professionals (QMHP) who provide mental health services and intensive case management to individuals living in long-term skilled nursing facilities. Individuals served are largely older adults, however there are no age restrictions or requirements for PASRR services.
PASRR Service Coordinators utilize a person-centered, trauma-informed, recovery focused approach to forge therapeutic relationships with racially and culturally diverse individuals to help them:
Define and facilitate desired life outcomes
Cultivate independent living skills
Process complex grief and adjustment issues
Build more effective coping skills to better manage their mental and behavioral health needs
PASRR Service Coordinators also:
Provide regular clinical assessment and evaluation of individual strengths, needs, and goals
Help clients locate and secure independent housing, financial assistance, transportation, or other resources necessary to achieving their goals
Participate in regular interdisciplinary team meetings with medical providers, family members, or other client supports
Identify potential service gaps and advocate with providers to ensure services are appropriate and adequate to clients' needs and preferences
The ideal candidate is someone who:
Is interested in working with older adults and/or individuals with significant mental health needs

Is self-motivated, independent, flexible, and appreciates variety in their work day
Communicates effectively and professionally

PASRR Service Coordinators work under general supervision of the unit manager with moderate latitude for the use of initiative and independent judgment.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education
Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.
License
Current and valid driver's license and a clean driving record is required.
Experience
1 year of experience working directly with individuals with IDD is required.
Hours Full-Time.
Workdays Monday - Friday.
Workhours 8:00AM - 5:00PM.
Salary $24.18 - $25.39 an hour, based on education.
