We are looking for Registered Nurses with experience in Home Health in Western Riverside County; Lake Elsinore, Canyon Lake, Murrieta, and surrounding areas.
Base Pay Range $65.00/day to $125.00/day
Actual pay will be based on skills and experience.
Essential Job Functions / Responsibilities
Complete an initial assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illnesses.
Regularly re-evaluate patient nursing needs.
Initiate the plan of care and make necessary revisions as patient status and needs change.
Use health assessment data to determine nursing diagnosis.
Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Include the patient and family in the planning process.
Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
Counsel the patient and family in meeting nursing and related needs.
Provide health care instructions to the patient as appropriate per assessment and plan of care.
Identify discharge planning needs as part of the care plan development and implement before the patient's discharge.
Communication
Prepare clinical notes and update the agency when necessary.
Communicate with the agency regarding the patient’s needs and report any changes in the patient’s condition; obtain/receive physician’s orders as required.
Communicate with community health‑related persons to coordinate the care plan.
Additional Duties
Ensure that arrangements for equipment and other necessary items and services are available.
Instruct, supervise, and evaluate home health aide care provided every two (2) weeks.
Position Qualifications
Registered nurse with current licensure to practice professional nursing in California. (REQUIRED)
Ability to perform lab draws.
Ability to perform wound care.
Experience in IVs (PICC line, Central, Midline, etc).
Tracheostomy management.
Graduate of a National League for the Nursing–accredited school of nursing. (REQUIRED)
Maintains a current CPR certification. (REQUIRED)
Must be a licensed driver with an insured automobile under state or organization requirements and good working order. (REQUIRED)
Minimum two years’ experience, at least one of which is in the area of public health or home care nursing, is preferred.
Self‑directed and able to work with minimal supervision.
Demonstrates excellent observation, problem‑solving, verbal and written communication skills, nursing skills per competency checklist.
Shows ability to organize and prioritize workload independently.
Management experience not required. Responsible for supervising home health aides.

While performing this job’s duties, the employee is regularly required to walk or stand for prolonged periods. The employee must occasionally be able to lift or move 50 pounds and transfer patients. Must lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
Job Types Per Diem
Pay $65.00 – $70.00 per visit (depends on experience); $100 – $125 SOC Oasis/visit.
