Job Title:
Geriatric Rounding Physician – Community-Based Practice
Compensation:
$225,000 – $250,000 annually

Position Summary:
We are hiring a
Physician (MD/DO)
who is passionate about senior care and enjoys
working outside of the traditional clinic setting.
You’ll round on-site at assisted living and memory care facilities, managing chronic conditions, coordinating care, and helping patients live fuller lives.
Key Responsibilities:
Conduct patient visits within assigned communities across the metro
Perform physical exams, diagnostics, and medication management
Lead care discussions and long-term planning with residents and families
Communicate regularly with advanced practice providers and facility staff
Address complex conditions with a preventative, person-centered approach
Qualifications:
Current MN license as a physician
Experience in geriatric medicine or primary care
Background in Hospitalist or senior community rounding preferred
Excellent interpersonal and leadership skills
Ability to travel locally to communities with reliable transportation
