Resource Nurse (Hospice) — Admissions, Case Management, Visit Nurse
Pay:
$90,000 – $130,000 DOE

Schedule:
Monday – Friday plus on‑call and weekend requirements.
Location:
Serving the Greater Twin Cities Metro Area (21‑County Service Area)

What You’ll Do
Admissions:
Complete hospice eligibility assessments, consents/education, medication reconciliation, immediate symptom plan, and safe‑start services.
Case Management (as assigned):
Own a patient panel—plan of care, symptom management, caregiver coaching, IDT coordination, timely documentation.
Visit Nurse/Crisis Support:
Provide PRN, routine, and urgent visits; execute standing orders; elevate and collaborate with the MD/NP swiftly.
Interdisciplinary Collaboration:
Partner with social work, chaplaincy, aides, and therapy to support patient and family goals.
Documentation & Compliance:
Chart accurately in the EMR; follow Medicare CoPs and agency policies.
What You’ll Bring
Required hospice background
from one of the following: prior hospice agency experience or high‑acuity settings such as ICU, palliative care, or oncology.
Strength in admissions interviewing, symptom triage, and family education.
Organized, decisive, and comfortable driving across the service area (reliable vehicle/insurance).
Professional, compassionate communicator who thrives in a dynamic environment.
