Reentry Coordinator (Grant Funded, Time Limited) 11116 This is a temporary, time-limited, grant funded position through June 30, 2026. The role performs professional and administrative work for the County's reentry program aimed at reducing recidivism and promoting successful community reintegration for justice-involved individuals.

Responsibilities

Conduct intake assessments and develop individualized reentry plans for Craven County inmates.

Coordinate service referrals for housing, employment, medical, behavioral health, substance use treatment, and social services.

Facilitate warm handoffs and closed-loop referrals to community partners.

Maintain regular communication with clients, service providers, and correctional staff.

Track participant progress and maintain accurate documentation for grant reporting.

Provide crisis intervention and support to inmates, as needed.

Participate in case review meetings and multidisciplinary team coordination.

Monitor outcomes and assist with program evaluation activities.

Ensure services are trauma‑informed, culturally responsive, and aligned with best practices.

Ensure compliance with COSSUP grant requirements.

Establish and maintain cooperative relationships with community partners and coordinate activities with other agencies and private organizations to enhance program activities and service availability.

Special Requirements Possession of a valid North Carolina driver’s license and reliable transportation.

Must be able to obtain background check clearance required for working in detention facilities.

Qualifications Graduation from an accredited college with an associate’s degree in human services or criminal justice and two or more years of experience in case management, reentry services, or related work, or an equivalent combination of related education, training or experience. A bachelor’s degree in social work, criminal justice, public health, or a related field is preferred.

Must be a North Carolina Certified Peer Support Specialist.

Knowledge, Skills, and Abilities

Comprehensive knowledge of community resources, criminal justice system, alternatives to incarceration, correctional treatment practices, local, state and federal laws, guidelines, and programs.

Basic working knowledge of inmate sentencing, pretrial conditions, and release.

Thorough knowledge of the principles and practices of case management and interviewing.

General knowledge of mental health and substance use disorders and available treatment and recovery resources.

Proficiency with Microsoft Office 365 (Word, Excel, PowerPoint, Outlook, Teams).

Ability to exercise sound judgment in routine and emergency situations.

Ability to work in a hazardous and confining working environment.

Ability to prepare and maintain accurate, thorough, and organized records.

Ability to communicate effectively both orally and in writing.

Ability to work independently and as part of a multidisciplinary team.

Ability to establish and maintain effective working relationships with a wide variety of community partners, court staff, and coworkers.

Ability to handle situations with inmates and the public tactfully and firmly.

Ability to multitask, prioritize duties, and manage multiple cases.

Ability to operate office equipment, computers and applicable software applications.

Ability to safely operate a motor vehicle.

Ability to attend work regularly and maintain confidentiality.

Additional Information Participation in disaster preparedness and response operations will be expected in the event of an emergency. Employees may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Other related duties as assigned.

Physical Requirements Must be able to perform light work exerting up to 20 pounds of force occasionally, up to 10 pounds frequently, or a negligible amount of force constantly to move objects. Vocal communication is required for expressing or exchanging ideas by means of the spoken word; hearing is required to perceive information at normal spoken word levels; visual acuity is required for compiling, reading, research, and inspecting finished written material. Must be able to physically perform the basic life functions of stooping, standing, walking, grasping, talking, and hearing.
