Our

Data Collection Coordinator

will be responsible for the issuing of the Important Information on Medicare Notification of Hospital Discharge Appeal Rights (CMS - 4105-F) to Medicare beneficiaries, patients enrolled in Medicare Advantage and other Medicare health plans under MA regulations, dual eligibles, and beneficiaries with Medicare as a secondary payer.

Responsibilities

Collaborate with Case Management, Physicians, Nursing, Patient Access, and Patient Relations to identify all patients who require an IM. Issue (within 2 calendar days of the patient's admission) an IM to the patient or the patient's representative and obtain a signature if the patient has not received an IM in the Patient Access Department/Admitting. File a copy of the IM in the patient's chart/record. Call the patient's representative (if not available) on the hospital telephone, and mail or fax the IM on the same day. Collaborate with the Nurse Case Manager to identify patients who will be discharged. Re-issue the IM 48 hours prior to the patient's discharge. Notify the Case Management Department and the Nurse Case Manager of the unit should a patient request an appeal of their discharge. Identify and participate in departmental P.I. activities, bringing any discrepancies to the attention of a supervisor. Qualifications

A High School diploma or equivalent. A minimum of one (1) year of healthcare or customer service experience. Excellent communication & interpersonal skills. Strong organizational skills. Ability to work independently with minimal supervision. Bilingual English/Spanish preferred.

Pay Range
