Sentara Home Care Services Greater Peninsula in Newport News, VA is hiring a full time Care Transition Coordinator. Onsite at hospital, M-F, holiday rotation Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care. Licensed RN, LPN, RT or BSW required. Proof of education as LPN, RN, RT or BSW required. 1 year of home care, discharge planning or case management experience required.
