Primary Work Location:
Multisite. This role will travel across the Bay Area as need to perform home health work.
Job Summary:
Cardea Health is a nonprofit healthcare organization that provides clinical services to vulnerable populations. We currently have an opening for a home health occupational therapist. Responsibilities: Choose to be an integral part of a team that focuses on rehabilitation of patients in their home. This may include providing care to individuals in unstable and/or non-traditional housing environments, such as shelters or unsheltered environments. The Occupational Therapist will be responsible for both evaluation and treatment of their patient caseload. Prior home health experience is preferred but not required.
Job Responsibilities & Qualifications Responsibilities:
Perform initial and ongoing clinical assessments to identify patient needs, determine level of functioning
Provide therapeutic treatments and evaluation of equipment needs as applicable
Evaluate outcomes of care with appropriate discharge planning
Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
Accept clinical assignments that are consistent with education and competence to care for patients
Complete appropriate documentation in a timely manner
Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs
Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate
Promote company philosophy and administrative policies
Demonstrate commitment, professional growth, and competency
Other duties as assigned
Minimum Required Skills & Qualifications:
Education: Bachelor of Science in Occupational Therapy from an American Occupational Therapy Association approved program
Licensure: Current California State License as a Occupational Therapist, current California Driver’s License
Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills Proof of current CPR. Knowledge of durable medical equipment
Transportation: Reliable transportation and valid and current auto liability insurance
Experience: 1+ years of Home Health occupational therapy experience (preferred)
Physical Requirements:
Frequent sitting or stationary work
Frequent standing, walking, kneeling, stooping, and ascending stairs
Frequent lifting and carrying up to 20 lbs
Occasional pushing and pulling
Occasional reaching high, low, and level
Work Environment:
In-person onsite
Occasional travel between sites
