Wellness Assistant | PACE Program
Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on “meeting individual needs through innovative care solutions,” and we serve people across multiple counties in northeast Kansas.
PACE Program The PACE program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Wellness Assistant Role A PACE Wellness Assistant supports older adults in PACE programs by promoting health, safety, and overall well‑being. They help with daily activities such as bathing and mobility, encourage healthy habits and exercise, assist with meals and social engagement, and monitor participants’ condition, reporting changes to the care team. Essentially, they provide hands‑on support to help participants stay independent and engaged while coordinating with nurses and other staff.
Responsibilities Provide direct support to participants in activities of daily living, including bathing, dressing, mobility, and transfers. Encourage participation in exercise and wellness programs. Assist with meal preparation and social activities. Monitor participants’ health status and communicate any changes to the interdisciplinary care team. Promote safety and independence through individualized care plans.
Qualifications Required Qualifications
High school diploma or GED
Basic caregiving or healthcare experience
Ability to assist with activities of daily living (bathing, dressing, mobility, transfers)
CPR and First Aid certification (or willingness to obtain)
Physical ability to lift and safely transfer participants
Ability to pass background checks and health screenings
Preferred Qualifications
Experience working with older adults
Previous work in PACE, assisted living, home health, or similar settings
Knowledge of elder care and dementia support
Strong communication and teamwork skills
