Overview The Case Manager/QIDP advocates for individuals with developmental disabilities by coordinating services and ensuring documentation and regulatory compliance. This role leads the development and implementation of service plans, maintains regular client and family contact, responds to crises, and monitors outcomes to support integration and well‑being.

Key Responsibilities

Lead the development and implementation of service plans.

Maintain regular client and family contact.

Respond to crises and monitor outcomes.

Facilitate team communication and conduct site visits.

Lead meetings, mentor staff, and ensure alignment with agency goals and standards.

Obtain QIDP Train the Trainer certification, Safety Care certification, and First Aid and CPR certification while employed.

Preferred Skills

Knowledge of theories, methodologies, technologies, and federal and state laws and regulations pertaining to developmental disabilities.

Ability to operate a personal vehicle as necessary for transportation needs of persons served.

Proficiency in agency computer equipment for word processing/data management, telephone systems, and other office equipment.
