Job Summary Provides services to Medical Assisted Treatment. Assists in developing, evaluating, and coordinating treatment plans and recovery services.
Core Competencies
Demonstrate competency in managed care systems and provider networks in the adult recovery model.
Demonstrate competency in case management procedures and resource allocation.
Demonstrate awareness of social determinants of health.
Adopt a culturally sensitive and non‑stigmatizing approach.
Display familiarity with models of care management.
Adopt a patient‑centered approach emphasizing self‑determination and shared decision making.
Maintain confidentiality and discretion.
Work effectively within an interdisciplinary team and communicate effectively.
Familiar with all personnel and program policies and procedures.
Multi‑task, prioritize, and work under pressure.
Effectively use electronic health records.
Job Requirements
Bachelor’s degree in a human service related field or Associate’s degree plus two years of behavioral health or substance use treatment experience.
Willingness to work unconventional hours (weekends, evenings, holidays).
Valid New Mexico driver’s license, current automobile insurance, and a clean driving record.
Willingness to use a personal vehicle for work when needed.
Must submit to LCDF required background check, TB screen, and drug testing.
