Overview
We're hiring a compassionate, self-motivated
Physical Therapist
to join a top-tier home health team serving patients across Lansing, Michigan. In this role, you'll deliver patient-centered care in the comfort of patients' homes, supported by dedicated technology, flexible scheduling, and career development resources.
Provide skilled physical therapy treatment to home health patients
Collaborate with interdisciplinary care teams to implement patient care plans
Maintain accurate, timely clinical documentation in the EMR system
Communicate with patients, families, and other healthcare providers
Utilize company-provided tablet and smartphone to complete documentation efficiently
Required Qualifications
Education:
Bachelor's degree or higher in Physical Therapy from an accredited institution
License:
Valid Michigan Physical Therapist license
Experience:
1+ years of healthcare experience
1+ years of experience with
any
of the following:
Electronic Medical Records (EMR)
TruChart EHR or other EMR systems
Certifications:
Valid Driver's License (DL)
Must
not
have applied to or worked for Residential Home Health & Hospice in the past 6 months
Must have
spoken with a Relode recruiter
about this position
