Role Description
We are seeking a dedicated Hospice Registered Nurse (RN) Case Manager for a full-time, on-site role in Dalton, GA. In this position, you will manage the care of hospice patients by coordinating and delivering individualized care plans. Responsibilities include conducting comprehensive assessments, providing nursing care, creating and managing discharge plans, and collaborating with interdisciplinary teams to ensure optimal patient outcomes. The RN Case Manager will also oversee case management activities, support families, and facilitate communication between patients, physicians, and care teams.
Qualifications
Proficiency in Nursing and Medicine, with a current RN license in the state of Georgia
Experience in Case Management and coordinating care plans
Knowledge of Discharge Planning and ability to develop and execute effective plans
Background in Utilization Management and resource optimization
Strong communication, compassion, and organizational skills
Ability to work collaboratively with interdisciplinary teams
Previous hospice or palliative care experience is preferred
Bachelor of Science in Nursing (BSN) is preferred
