Position Overview
SA Nurses is rapidly growing, and we are currently hiring Home Health Nurses. Our vision is to deliver the highest quality of health care to all people; we strive to raise the standard of care provided to our patients and therefore are seeking creative, compassionate, caring, and respectful professional nurses. Home Health nurses will be responsible for providing at‑home nursing care for patients, often as follow-up care after discharge from a hospital or from a rehabilitation, long‑term care, or skilled nursing facility.
Responsibilities
Assess patients' condition during every visit and chart observations.
Perform evaluation tasks, including vital signs and medication review, Oasis assessment, wound care and IV therapy.
Dress or redress open wounds and assess progress of healing.
Collect data and prioritize problems and needs.
Supervise CNAs and LVNs assigned to your caseload.
Document to company and compliance standards.
Educate patients and their families on proper home health care strategies and procedures.
Coordinate with occupational therapists, physical therapists, physicians, and others involved in the patient's care plan.
Make recommendations for devices or tools that might improve the patient's quality of life.
Listen to the patient and respond to concerns or requests.
Qualifications Some key qualifications for this position rely on a variety of skills, including

time management skills
