Job Summary
Position: Physical Therapist (PT) – Outpatient Care
Location: Lone Tree, CO
Hourly Wage Estimate: $37.26 - $55.91 / hour.
This role provides high‑quality, patient‑centered care for an assigned group of patients in coordination with physicians and the patient care team. The PT performs patient assessments, develops individualized care plans, and provides the majority of the therapy needed to support recovery.
Responsibilities
Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
Assess and evaluate therapeutic/rehabilitative/functional status and participate in development of the total plan of care.
Provide purposeful and skilled therapeutic care using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
Prioritize caseload, determine daily staffing needs, and coordinate assignments and hand‑offs.
Meet weekend work requirement of 2 days per month.
Qualifications
Graduate of an American Physical Therapy Association accredited school with a BS, MS, or Doctorate degree in Physical Therapy.
Active Physical Therapy license in the State of Colorado.
Current Basic Life Support (BLS) certification for HealthCare Providers (American Heart Association).
Successful completion of student clinical rotation or work experience in at least one of the following settings: acute care hospital, long‑term acute care facility, skilled nursing facility or acute rehabilitation setting.
(LPT) Licensed Physical Therapist.
Certified Lymphedema Therapist credential required for the oncology population.
Ability to work two weekend days per month.
