Registered Nurse
for the
Program of All-Inclusive Care for the Elderly (PACE)
in
Downtown Los Angeles, CA . In this role, you'll provide high-quality clinical nursing care across homes, adult day centers, SNFs, and clinics while collaborating with an Interdisciplinary Team (IDT) to support frail older adults.

Required Qualifications
Associate Degree or higher in Nursing
Registered Nurse (RN)
Licensed in California
American Heart Association BLS Certification (AHA BLS)
Valid CA Drivers License , personal transportation, clean driving record, and active auto insurance
2+ years of experience in all of the following:
Geriatric Care
Adult Complex Population
Prior CNA, CMA, or LVN experience (clinical foundation)
Lives within
10 miles of Downtown Los Angeles
1+ years of experience in
PACE
or senior care programs
Bilingual in Spanish (preferred but not required)
Conduct comprehensive, face-to-face clinical assessments across a variety of care settings
Develop and manage personalized care plans in collaboration with medical providers and IDT
Administer nursing interventions, medications, and therapeutic procedures
Provide triage, case management, and transition-of-care coordination
Educate participants, caregivers, and team members
Participate in a rotating
after-hours call schedule
Champion whole-person care and proactive health outcomes
Schedule
Full-time, Monday-Friday

Rotating after-hours triage (remote support system in place)
