Role Overview As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes in accordance with physician orders and applicable laws, regulations, and agency policies. You will coordinate a total plan of care with other health care professionals and ensure continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, and community resources.

Primary Responsibilities

Clinical Competence:

Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.

Supervision:

Supervise care provided by home health aides and licensed practical/vocational nurses, provide instruction, assign tasks, and conduct required supervisory visits in accordance with state and federal regulations.

Documentation and Care Delivery:

Provide high‑quality clinical services within the scope of practice and infection control standards, complete comprehensive assessments (OASIS) including medication reconciliation, and document patient visits per policy and payer requirements.

Quality Assurance:

Conduct initial and/or comprehensive nursing evaluation visits, ensure patients meet home health eligibility and medical necessity guidelines, and communicate timely and effectively with agency staff, physicians, patients, and families.

Teamwork & Continuous Improvement:

Follow direction from the Clinical Director and Executive Director, assist in orientation of new personnel and serve as a preceptor, participate in survey readiness activities, perform on‑call and weekend rotations as needed, and support the agency’s utilization management model.

Required Qualifications

Current and unrestricted RN licensure in state of practice.

At least 1 year of home health experience.

Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

Current CPR certification.

Ability to function in any home situation regardless of client characteristics.

Preferred Qualifications

Ability to work independently.

Strong communication, writing, and organizational skills.

Pay Range and Compensation Annual total cash target pay:

$64,100 – $141,500.

Per visit point pay:

$36.98 – $81.63.

Hourly rate:

$30.82 – $68.03.

Compensation is based on productivity points for visits and hourly pay for non‑visit activities. Benefits include a comprehensive benefits package, recognition programs, equity stock purchase plan, and 401(k) contribution.
