Job Summary Deliver compassionate, patient-centered hospice care that preserves dignity and comfort for patients and families.
Job Specific Duties
Conduct initial and ongoing assessments and develop, implement, and revise individualized hospice plans of care in accordance with physician orders and regulatory timelines
Provide skilled nursing services including pain and symptom management
Complete accurate and timely clinical documentation to support compliance with hospice Conditions of Participation
Notify physicians of patient condition changes
Process orders, certifications of terminal illness, recertifications, and discharge or transfer summaries
Provide direct patient care and on‑call coverage as assigned
Respond promptly to urgent patient or caregiver needs, including crisis intervention
Supervise and train LPNs and HHAs, including conducting required supervisory visits
Retain accountability for delegated tasks
Educate patients and families on the hospice philosophy, end‑of‑life care, pain and symptom control, infection control, emergency preparedness, and patient rights
Coordinate care with the interdisciplinary group (IDG)
Participate in IDG meetings, care conferences, QAPI activities, and peer review
Maintain current licensure, CPR certification, and complete required continuing education in compliance with state and federal regulations
Other Duties
Provide care and services in accordance with infection control, safety, and risk prevention protocols
Accurately document clinical interventions, care coordination, and communication in a timely manner
Participate in interdisciplinary meetings, case reviews, and continuing education opportunities
Maintain compliance with HIPAA and all company privacy and confidentiality standards
Represent the organization positively and professionally in patients’ homes and community settings
Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities
Provide courteous, timely, and professional customer service to patients, families, and partners
Exhibit adaptability to changing responsibilities and flexibility in completing assignments
Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms
Adhere to all organizational policies, including job descriptions, mission, and the Employee Handbook
Communicate and interact professionally and respectfully with others to support team goals
Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the company

Requirements Qualifications
Graduate of an accredited school of professional nursing
Current, unencumbered RN license in the state(s) of practice
Two years of nursing experience preferred
Hospice or palliative care experience a plus but not required
Ability to exercise sound clinical judgment
Ability to provide compassionate end‑of‑life care in varied settings
Strong communication and interpersonal skills
Sensitivity to diverse cultural, spiritual, and emotional needs
Basic computer proficiency
Willingness to learn EMR systems and hospice‑specific documentation requirements
Current CPR certification
Commitment to compliance with Medicare and Medicaid Hospice Conditions of Participation, state regulations, and agency policies
Reliable transportation
Valid driver’s license
Auto insurance for home visits
Physical Requirements
Must be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.
