About the job LVN/LPN - Behavioral Health (PERM) Vermont
Job Title: LVN/LPN - Licensed Vocational Nurse / Licensed Practical Nurse Responsibilities
Administer medications and treatments as prescribed by the physician. Monitor and record patient vital signs. Assist with patient assessments and care planning. Collaborate with the healthcare team to ensure comprehensive patient care. Provide emotional support to patients and their families. Maintain accurate and up-to-date patient records. Adhere to all facility policies and procedures. Share information appropriately with members of the multidisciplinary team and other hospital department representatives through verbal reports and written documentation. Participate in pre‑admission evaluation as requested by Program Manager and complete Admission Nursing Assessment on all new admissions to C/A Residential I & Partial Programs (Including ARCC). Conduct monthly medication renewals and review daily reports. Follow up on medical tests and labs, oversee completion of admission paperwork, and complete the medication portion of aftercare forms.
