Hourly wages are competitive and are based on region, qualifications, experience, and fit. RNs: $34 – $35 per hour LPNs: $26 – $28 per hour Requirements & Qualifications

Active LPN or RN license in good standing in Ohio. Minimum of 1 year of experience in home healthcare, acute care, or equivalent nursing setting. Completed HCN’s written skills test or competency evaluation. Compliance with all state‑required training and certifications for the area served. Current CPR and First Aid certifications. Ability to perform all main physical tasks safely. Strong nursing, patient care, safety, compliance, and confidentiality background. Excellent attendance record. Good interpersonal skills and compassion. Ability to read, write, and follow a written Plan of Care with detail orientation. Reliable, honest, punctual, articulate, compassionate, detail‑oriented, accountable, friendly, respectful, self‑driven, problem‑solver, compliant, confidential. Minimum age 18. Valid driver’s license/ID and reliable transportation. Consent to a background check. Desired but Not Always Required

3‑5 years of paid experience in a home health or caregiving setting. Paid experience with a Home Health Agency. Specialized care experience (G‑tube, trach, vent, infants, disabilities, family caregiver support, etc.). EMR/EHR proficiency, especially ContinuLink. Willingness to promote HCN within community and networks for bonuses. Job Summary

A successful PDN performs skilled nursing functions in accordance with federal, state, and local laws and agency policies, delivering care delegated by the RN Case Manager. Essential Functions/Areas of Accountability

Provide professional nursing care under RN direction and report status changes. Perform skilled nursing visits following the RN case plan. Recognize and report client condition changes to RN, Case Manager, and physician. Communicate timely via verbal and documentation. Conduct personal care services, assessment, medication monitoring, client teaching, emergency intervention, and nursing services within scope. Perform venipuncture as permitted for licensed LPNs (M‑IV). Promote personal safety and safe environment. Adhere to safety/infection control practices. Respond to unsafe situations. Use equipment safely. Recommend safety improvements. Follow agency policies, professional and community standards. Accept responsibility for professional role. Comply with applicable laws and standards. Attend required meetings. Maintain confidentiality. Refrain from discussing client or staff identities, conditions, or circumstances publicly. Participate in quality improvement program. Perform other related duties as necessary. Essential Physical Demands

Frequent: standing, walking, bending, stooping. Occasional: squatting/kneeling, balancing, sitting. Pushing/handling weight: 10‑20 lbs occasionally, 20‑30 lbs infrequently.
