Essential Functions

Utilizes RN process as a framework to focus the activities of the healthcare team on the achievement of optimal outcomes, resource utilization, clinical expertise, and improvement strategies.

Interacts with patients, professionals, and the community to achieve continuity of care, coordination of services and to document plans of care across multiple care settings.

Conducts comprehensive “all-system” needs assessment for identified patients; matches needs with appropriate disease‑management and clinical goal expectations.

Develops and maintains accurate case records for each referred customer/patient.

Documents fully and accurately using computer databases and documentation systems.

Maintains knowledge of reimbursement criteria and documentation necessary for Medicaid, Medicare, and Managed Care.

Demonstrates leadership in nursing practice and evaluates own practice in relation to professional standards and regulations.

Physical Requirements

Walking, standing, lifting, reaching, bending, stooping.

Lift a minimum of 35 pounds shoulder‑high.

Travel between locations required.

Verbal and written communication in English.

Intact sight, hearing, finger dexterity, critical thinking, concentration.

Respond quickly to changes in assignments.

Occasional exposure to dust, fumes, and chemicals.

Education, Experience and Certifications

BSN required, Master’s preferred.

Current RN license or temporary license as a Registered Nurse Petitioner.

Minimum two years experience in health care, including case management/discharge planning in Acute care, Home care, LTC, Physician Office or Managed Care.

Professional certification within 3 years of hire date per departmental protocol.

Clinical competence in disease management and case‑management principles.

Excellent interpersonal communication, negotiation, problem‑solving, organizational and time‑management skills.

BLS required.

Compensation

Base compensation within the pay range based on qualifications and experience.

Premium pay such as shift, on‑call, and other.
