Responsibilities:
Carry out preventive and rehabilitative nursing procedures in accordance with the established Plan of Care.
Administer medications and treatments as prescribed by physicians and ordered within the Plan of Care.
Gather and document patient data, including vital signs, medication reconciliation, medical history updates, and observable changes in patient condition.
Monitor patients for changes in status and promptly report findings to the Registered Nurse (RN) and Physician to support care plan adjustments as needed.
Collaborate with RNs, physicians, nurse practitioners, and interdisciplinary team members to support continuity and quality of care.
Respond to patient, caregiver, and clinical team inquiries related to treatment, comfort, and daily care needs.
Provide education, support, and reinforcement of care instructions to patients and caregivers as directed by the Plan of Care.
Assist with discharge planning activities by identifying patient needs, concerns, or barriers and communicating them to the RN.
Qualifications:
Must hold an active Nursing license in the State where the position is based.
Preferably 2+ years of nursing experience, with a focus on geriatrics.
Nursing degree from an accredited institution.
Excellent bedside manners and exceptional communication skills.
Working knowledge of healthcare laws and regulations.
Maintain current BLS CPR certification.
Possess a valid driver's license and own a reliable, insured vehicle.
