Job Description SOCIAL WORKER IV- Patient Services Department

Duties Summary:

Social Workers seek to improve the overall quality of life of people within the context of their environment through the remediation and prevention of their problems, and by improving accessibility, accountability and coordination of service delivery among a variety of professionals and/or agencies tasked with providing appropriate services to patients. Social Workers establish working relationships of mutual respect and trust with the patients and families served, and utilize a variety of helping interventions to enable these patients to function as effectively as possible.

MINIMUM QUALIFICATION REQUIREMENTS

To qualify, you must meet all of the following requirements. Please note that unless specifically indicated, the required education and experience are credited based on a 40‑hour workweek.

EDUCATION:

Graduation with a bachelor’s degree or master’s degree in social work from a program of study accredited by the Council on Social Work Education, or a doctoral degree in social work from a college or university accredited by the Western Association of Schools and Colleges, or a comparable regional accreditation body.

EXPERIENCE REQUIREMENTS:

Applicants must have had progressively responsible experience of the kind and quality described below or an equivalent combination of training and experience.

BSW: 2 years specialized experience, 1 year supervisory experience, 0 years administrative experience, 0 years total experience.

MSW: 2 years specialized experience, 1 year supervisory experience, 0 years administrative experience, 0 years total experience.

PhD SW: 2 years specialized experience, 1 year supervisory experience, 0 years administrative experience, 0 years total experience.

Specialized Experience:

Progressively responsible professional work experience which involved helping individuals and their families find satisfactory ways of identifying their problems, coping with their conditions, and functioning effectively within their environments. Depending on the employment setting and the kinds of clients or patients served, duties may vary, although typically the work includes identification and evaluation of the client’s or patient’s problems and needs; development of a service or treatment plan; initiation and implementation of the service plan, either directly or through other providers; monitoring of services; and evaluation/assessment of the client’s progress, with amendments to the plan made as appropriate.

* Rates listed for paid holidays, vacation and sick days are for 1.0 Full Time Equivalent (FTE) positions assigned to 8 hour shifts. Accruals for these benefits are otherwise prorated based on FTE.

Pre-Employment Health Requirements

Offers of employment will be contingent on successfully passing a pre‑employment examination, which includes a drug screen and other regulatory medical requirements such as a tuberculosis (TB) screen. The costs for drug screening and blood draw for QuantiFERON and Titers shall be borne by the Hawaii Health Systems Corporation.
