About The Role As a social worker, you’ll work with a care team that includes a physician or nurse practitioner, to assess mental and behavioral needs, develop treatment plans, and collaborate with other medical staff to ensure comprehensive care for a unique patient population.
Responsibilities
Oversees and provides direct and team based behavioral health care, may include supervision depending on license.
Facilitates care coordination and clinical collaboration.
Ensures quality care and risk mitigation.
You will be primarily based at our Mattina location, 300 Niagara Street, Buffalo, NY 14201, and will travel to other Neighborhood sites in Western New York as needed.

Education and Skills
Master’s degree in social work and a strong desire to work with an urban population; must possess a thorough knowledge of health issues affecting the population throughout the lifecycle.

Excellent written and verbal communication skills.
What We Offer Compensation:
LMSW - $62,000-$66,000; LCSW - $67,000-$72,000 annual salary (based on a full-time, 40 hour work week). Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
