Citadel At Casa Scalabrini is seeking a Licensed Practical Nurse (LPN) to join their team! The LPN assumes responsibility and accountability for a group of residents for a shift of duty.
Duties and Responsibilities
Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations
Provides and receives nursing reports upon reporting in and ending shift duty hours
Ensures assigned work area (e.g., nurse station, medication cart, medication room, etc.) and resident care rooms are maintained in a clean and sanitary manner
Directs and supervises the day‑to‑day certified nursing assistant services for the assigned unit to assure care is rendered in accordance with current standards, guidelines and regulations
Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care
Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy as ordered within license limitations, and provides respiratory care (e.g., oxygen, nebulizer treatments)
Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record ("MAR"), treatment/care plans as required
Prepares and administers medications and performs treatments as ordered by the physician
Charts nurse’s notes in an informative and descriptive manner reflecting the care provided and the resident’s response
Completes required forms upon admission, transfer and/or discharge
Ensures the resident’s environment is free from accidents and hazards, addressing potential safety risks promptly
Makes written and oral reports/recommendations concerning shift activities as required
Ensures all aspects of resident care plans are implemented and maintained; recommends care plan changes to the IDT per professional standards
All other duties as assigned
Skills and Abilities
Excellent written and verbal communication skills in English
Ability to multi‑task and remain calm under stress
Analytical and problem‑solving skills
Understanding and following infection control standards, policies and procedures
Adaptability to a flexible work schedule and occasional overnight travel
Basic computer knowledge and aptitude to learn company applications
Education and Experience
Current CPR certification required
Current license to practice as a Licensed Practical Nurse in the state
One year of experience preferred
Long‑term care experience preferred
Physical Requirements
Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, with or without mechanical devices required
Limited potential for exposure to environmental hazards; understanding and adherence to company safety standards and protocols required
