Advanced Practice Nurse (PRN)
Ascend Health is seeking an Advanced Practice Nurse to join our South NJ hospice team on a PRN basis. The Nurse will conduct re‑certification visits, coordinate care, and support our interdisciplinary team.
Responsibilities
Conduct re‑certification visits to patients on hospice in accordance with local, state and federal regulations.
Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director.
Facilitate access to appropriate support models of care (e.g., hospice, rehabilitation, on‑going aggressive treatment).
Consult with the referring physician, acute care team, and other interdisciplinary members to contribute to the patient's plan of care.
Prepare and maintain accurate patient records, charts, and documents.
Assist in the development of clinical practice guidelines and standards to support quality palliative care as needed.
Ensure collaboration with patients, caregivers, medical directors, and members of the IDT.
Education
Master's Degree in Nursing.
Qualifications
Valid Nurse Practitioner license.
2–4 years of experience working within hospice or a related field.
Valid driver’s license and reliable transportation.
Willingness to travel.
