Full time, 40 hours per week, Monday through Friday.

Purpose Provide patient‑centered, high‑quality care management services in a timely manner to clients with complex chronic conditions by navigating systems such as primary medical, specialty and behavioral health care, as well as social services.

Responsibilities

Provide comprehensive care management services including outreach and engagement; assessment/reassessment; development and implementation of individualized care plans; consult with primary care physicians and other care providers; identify crisis intervention plans.

Coordinate care and health promotion by collaborating with service providers and health plans to obtain medically necessary care; share crisis interventions and emergency information; link to services; educate patients; support self‑help recovery and self‑management; conduct case reviews; advocate for services; assist with scheduling; monitor and support patients at appointments; intervene in crises as needed.

Provide comprehensive transitional care by following up on admission and discharge notices; facilitating discharge planning; notifying service providers; linking with community supports; conducting post‑discharge follow‑up.

Support members and families through care planning, consultation and education; conduct patient/family meetings; refer to peer supports, support groups, social services and other programs; collaborate with community providers.

Refer to community and social support services; identify and link to needed services; ensure access to medical, behavioral health, substance abuse, and other community‑based services.

Complete documentation promptly, including notes, assessments and tracking tools.

Adhere to company policies, corporate compliance, and code of conduct; maintain required certifications and licenses.

Attend required trainings, in‑services, and departmental meetings.

Perform other related duties as assigned.

Qualifications

Associate’s degree in Human Services and 2 years of related experience, or Bachelor’s degree with 1 year of related experience in public or community health serving low‑income and culturally diverse populations.

Computer literacy required.

Valid New York State driver’s license, clean driving record, reliable vehicle with proof of insurance.
