Independently practices social work and receives limited consultation, functioning as a member of the multidisciplinary healthcare team to provide case management, therapeutic crisis intervention, biopsychosocial assessment, clinical evaluation, therapy, counseling, referral, treatment planning, and other clinical social work services within their practice area.

Principal Responsibilities And Tasks

Completes specialized assessment(s) in a focus area/practice setting as needed and provides feedback regarding the enhancement of assessment tools.

Provides psychosocial guidance and drives conversations about Advance Care Planning with the healthcare team.

Provides advanced ongoing and crisis care to facilitate a therapeutic and safe environment for patients and serves as a professional resource to other members of the care team.

Suggests/initiates PI projects to evaluate program and outcome effectiveness.

Demonstrates advanced knowledge and skillsets appropriate to the age, culture, ethnicity, spirituality, and individual diagnostic needs of the persons served.

Demonstrates advanced knowledge of relevant resources to share with team members and initiates networking based on program gaps; serves as a lead contact for complex patient cases.

Empowers patients and the practice area with problem-solving and decision-making around personal, health, social, educational, vocational, financial and other interpersonal concerns within the context of discharge planning, clinic visit, or ongoing behavioral health assistance.

Functions independently with daily work and models acceptance of peer feedback; provides consultation and assistance with challenging cases to other social workers.

Conducts clinical reviews of assessments, planning, interventions, and overall plan of care to provide peer feedback.

Demonstrates advanced ability to provide education to patient and peers that aligns with the NASW Code of Ethics.

Demonstrates ability to independently advocate for self and team members.

Participates in a council and/or a committee within the healthcare system.

Participates in conversations about unit practice and provides recommendations for improvement.

Qualifications

Master’s degree in Social Work accredited by Council on Social Work Education (CSWE).

LCSW‑C (Licensed Certified Social Worker‑Clinical) licensure from the Maryland Board of Social Work Examiners.

Minimum three (3) years of post‑Master’s experience.

Knowledge, Skills And Abilities

Demonstrates skill in clinical social work, case management practice areas; completing clinical social work functions such as clinical assessment, treatment planning, clinical intervention, case management, psychoeducation, referral, psychotherapy, counseling, diagnosis of mental health disorders, advocacy; effective critical thinking; communications written and oral; age appropriate interpersonal interactions.

Ability to communicate and collaborate effectively with internal and external stakeholders; assess, adapt, and calmly respond to changing and/or crisis environments; make independent decisions consistent with current policies, procedures, and ethical standards; prioritize work assignments and manage time effectively; assist in data analysis; demonstrate innovative thinking, integrity, and accountability; act as a model for peers.

Compensation

Pay Range: $35.08 – $52.64 per hour (annualized may vary).
