RN Navigator Oncology

Job Overview The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve optimal clinical outcomes for patients and their caregivers.
Responsibilities
Coordinate patient care and services across the continuum.
Lead multidisciplinary team meetings to identify and eliminate barriers to care.
Promote effective resource utilization to achieve optimal clinical outcomes.
Provide psychosocial assessment and clinical support.
Qualifications
BSN required.
Three years of RN experience, with at least one year in Oncology.
Strong psychosocial assessment and clinical skills.
RN license.
BLS certification from the American Heart Association.
Physical Requirements Medium work: Exert/lift up to 50 lbs occasionally, 20 lbs frequently, 10 lbs constantly.
Compensation and Benefits Pay range: $34.14 – $54.46 per hour.
Relocation assistance: up to $8,000 (depending on location).
Sign‑on bonus: $20,000 for external applicants with at least six months of direct RN experience.
Location The position is located at St. Francis Medical Center, Penrose – St. Francis Health Services, Colorado Springs, CO. The facility offers comprehensive oncology care, a Level III NICU, and advanced surgical and critical‑care services within a large, magnet‑designated acute‑care environment.
