Responsibilities
Provide 1:1 care for clients following their plan of care while maintaining dignity and independence.
Administer medications, feedings, oxygen, and ostomy care as needed.
Accurately document nursing actions, communicate with family, case manager, and physicians, and update progress toward established goals.
Coordinate or order supplies, medications, and feedings to ensure adequate availability.
Ensure all patient needs are met in accordance with physician instructions.
Qualifications
LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
Valid driver’s license, safe driving record, and proof of car insurance.
CPR professional certification.
1+ years of nursing experience preferred.
Home Health care experience preferred.
