Frontier Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community. Schedule: Monday - Friday, 8 am - 4:30 pm. Territory: Tulsa. In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Responsibilities
Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.
Nurturing Direct Care: Coordinate and provide direct nursing support to patients, ensuring their comfort and quality of life remain paramount.
Nursing with Heart: Deliver top‑notch nursing care rooted in proven principles and techniques, helping to ease pain and bring solace to those under our care.
Educator and Advocate: Empower patients and families through education, encouraging their active participation in creating personalized care plans that align with their goals.
Call of Compassion: Share responsibility for call duties, being there when it matters most.
Qualifications
Hold current unencumbered license as a Registered Nurse.
Minimum one (1) year experience as an RN in a medical‑surgical/acute care setting. Hospice experience is a plus.
Computer proficient in typing and various programs, including background in EMR.
Must possess and maintain current CPR certification if required by state.
Current driver’s license, vehicle insurance, and access to dependable transportation.
$76,000 - $84,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
